The multi professional team at Sobell House includes a Social Worker/Care Manager, who supports inpatient unit patients and their families, friends and carers directly, and provides links with teams in the community for Day patients and Out patients.

Practical care is arranged for patients returning home after a stay on the inpatient unit, to allow them to remain at home, living life alone or with their families, as fully as possible, for the duration of their illness.

Twenty-four hour care in a residential setting is arranged for patients if this is needed, and advice given on funding.

Advocacy and support is available to those who may be experiencing problems with housing, finance, schools, or other psychosocial needs arising from illness, or made worse by illness as a further difficulty in life.

Practical support is essential in achieving a sense of safety, possibility and security in a changing situation, and every effort is made to extend the care and support offered on the inpatient unit into the home setting.


 

Benefits Advice
Sometimes illness can cause financial problems, with possible loss of income, or extra expenditure. It can be difficult at such a time to deal with all the bureaucratic requirements to get the benefits you may be entitled to.

The welfare adviser is able to help patients, carers and families with information and advice about the benefits that are available. Help is also available with filling in forms, and sorting out problems that may arise in the complicated maze of benefits and tax credits.
