C O M M U N I T Y  A N D  H O S P I T A L  T E A M S

The Community and Hospital Palliative Care Teams offer advice and support to patients who are faced with life threatening illness, as well as to their families and carers, in the Oxford Radcliffe Hospitals NHS Trust and in the community. We also advise and support the medical, nursing, allied health professionals and other staff in the ward and primary care teams.

We take into account the full range of patient needs. We offer advice on managing complex symptoms, as well as emotional and spiritual support, rehabilitation, and information on the palliative care resources available.

Education, research and audit are important components of our service. The teams provide a wide range of education on hospital and community palliative care topics, both locally and nationally. The teams comprise medical, nursing, occupational therapy, and administrative staff, with support from social work and chaplaincy.

 

The Hospital and Community Palliative Care Teams cover the John Radcliffe Hospital, Churchill Hospital and Radcliffe Infirmary, and the Oxfordshire Primary Care Trust, working closely with other palliative care providers in the county (Katherine House Hospice in Banbury and Sue Ryder Care in Nettlebed).
